CONSENT
FORMS
No matter what event is going on, all participants must have a signed
consent form on file. Depending on your level of participation with
MAX, we have two different consent forms.
All-Year
Consent Form
This is the form you need to fill out if you are going to be participating
with MAX for the entire year. Once you fill this form out, you won't
have to fill another one out for the remainder of the school year.
One-Time
Consent Form
If you are just attending one MAX event, then this consent form is the
one for you. Maybe you are coming as a guest or just want to check us
out, complete this form and you will be covered for one MAX event.
